Healthier Together

The Healthier Together programme relies upon patients and healthcare professionals working together to improve how local healthcare is delivered. The website provides advice for parents, young people and pregnant women, and clinical resources to support healthcare professionals – which means that your child is likely to receive consistently high-quality care, irrespective of which healthcare professional they see.

Help if you’re homeless: domestic abuse

If you aren't safe in your home because of violence, abuse or threatening behaviour, you can get help with a safe place to stay or support to stay in your home safely.  For more information on how to access help click on the link to Shelter

Social Prescribing Service - We're Here for You

Our Health Link Worker can support and assess your needs and provide you with a social prescription – a service or activity – that will benefit your health and well-being. The Health Link Worker will arrange an appointment with you to discuss what types of support would benefit you. With your consent they will make suggestions and/or refer you to other services that may benefit your health and well-being and lifestyle. You will be offered support tailored to your own situation and the health link worker will keep in touch with you to review your progress. Our service works across the City with SHU Primary Care Network in Sheffield.

To access the service:

Contact your GP and ask for a referral for Social Prescribing. We look forward to working with you. 

Shipshape

ShipShape work to tackle health inequalities and to empower people  to achieve their own health and wellbeing goals.  They run free and low-cost group and one-to-one sessions, both at our Centre and in outreach venues and develop positive cooperation with partner agencies.  To see more of about their what is going on click ShipShape
